Comments:
An unannounced monitoring inspection was done today, September 24, 2019, beginning at 2:45 PM and ending at 5:00 PM. There were 12 children in care today with 2 staff providing care and supervision. There were no medications onsite during the inspection. The playground area and all rooms that are used for care were inspected, as were five children's records and all participating staff records. All staff records were reviewed at another location on September 16, 2019. The inspector looked at first aid and emergency supplies, drill documentation, and required health and fire inspections. The children were observed at arrival, during outside play, preparing for and eating snack, and during homework time. There were discussions about children's records and playground safety. Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion with staff, the center failed to ensure that the center shall maintain and keep at the center a separate record for each child enrolled which shall contain all elements as required by the standards.

Evidence:

1. Children (1, 2, 3, 4 and 5) did not have addresses for custodial parent(s) or guardian(s). The form did not have an option available for a parent or guardian to list an address that may differ from the child's address.
2. Children (4 and 5) did not have complete addresses listed for either of the two emergency contacts. The addresses did not include cities or states for the emergency contacts.

Plan of Correction: Staff will obtain this information from parents.
