Comments:
An unannounced monitoring inspection was done today, September 25, 2019, beginning at 10:00 AM and ending at 2:30 PM. There were 42 children in care today with ten staff providing care and supervision and two other professional staff. There were three medications reviewed onsite during the inspection. The playground area and all rooms that are used for care were inspected, as were five children?s records and five staff records. The inspector looked at first aid and emergency supplies, drill documentation, and required health and fire inspections. The children were observed during dance and gross motor play, preparation for lunch, lunch, and preparation for nap. Staff were observed during instructional times, hand-washing and diapering practices, and teaching. There were discussions about staff records, playground safety, and first aid kits. Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion, the center failed to ensure that all children's records contained all elements as required by the standards.

Evidence:

1. Child (3) was missing an applicable work phone number and place of employment of each parent who has custody.
2. Child (5) was missing an applicable work phone number for place of employment for one parent who has custody.

Plan of Correction: Staff corrected both files during the inspection.

Standard #: 22VAC40-185-70-A
Description: Based on record review, the center failed to ensure that staff records shall be kept for each staff person.

Evidence:

1. Staff records for staff (1 and 2) did not include a name, address, and telephone number of a person to be notified in an emergency which shall be kept at the center.
2. Staff records for staff (1, 2, 3, 4, and 5) did not include information, to be kept at the center, about any health problems which may interfere with fulfilling the job responsibilities.

Plan of Correction: Staff will ensure that this information is obtained.

Standard #: 22VAC40-185-510-G
Description: Based on observation and discussion, the center failed to ensure that the center only administers only those drugs that were dispensed from a pharmacy and maintained in the original, labeled container.

Evidence:

Medication (1) was contained in a cloth zipper bag without any labels and/or labeled container dispensed from the pharmacy. The LI asked staff if there was a label from the pharmacy and staff stated that there was not.

Plan of Correction: Staff corrected this violation during the inspection.
