Comments:
An unanncounced monitoring inspection was conducted between 8:00am and 9:00am. At the time of the tour there were 33 children in care with 3 staff in the program. Children were observed cleaning-up from an activity to prepare to go to class. The children were also observed in-line to get
their breakfast. A sample of 3 staff and 3 children's records were reviewed, in addition to 3 medications.

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on a record review the center did not provide the licensing representative an annual fire inspection report.
Evidence:
The director was unable to provide the center's last fire inspection.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
The director will check with the school to see if they have a copy of a fire inspection and will also follow-up with the fire marshal.

Standard #: 22VAC40-185-350-C
Description: Based on observation and interview, the center did not ensure when children are regularly in ongoing mixed age groups, the staff -to-children ratio applicable to the youngest child in the group shall apply to the entire group.
Evidence:
When the inspector arrived at the center around 8:20 am, there were 33 children, with the youngest child 3 years-old with 2 staff present. The ratio for 3 year-old children is 1 staff per 10 children. The center regularly operates with a group of children of mixed ages. The inspector asked staff #3 how many children were present and she stated, "There are 33 children." The inspector asked where the third staff member was. She stated, "she stepped out." Staff #3 left the gym, leaving staff #2 alone with the 33 children. She returned in about 1-2 minutes with the third staff #1. Staff #1 confirmed the class had children as young as 3 years present.

Plan of Correction: The director provided the following corrective action during the exit interview:
The director stated she stepped out of the gym to work on some files that are located in another room. The director stated she will ensure ratios will be maintained at all times and will compete filing and record keeping during times where she is not needed to maintain ratio during the program.
