On July 10, 2019, the department received a complaint regarding forbidden actions, behavioral guidance, and supervision of children. The licensing inspector conducted an unannounced complaint inspection with outside agencies on 7/17/19 at the facility from approximately 9:00 AM until 9:50 AM. On 2/20/19 the licensing inspector returned to the facility and conducted an additional inspection from approximately 10:40 AM until 1:50 PM. Interviews with staff were completed, observations were made and documentation was reviewed. The investigation continued off site through a review of documentation on 8/27/19 and 9/13/19; upon receipt of documentation from outside agencies. On 9/25/19 the licensing inspector returned to the facility and reviewed documentation from approximately 10:00 AM until 10:50 AM. A phone interview was conducted on 9/26/19. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection unrelated to the allegations. See the violation notice for more details. Standard #: 22VAC40-185-240-C Complaint related: No Description: Based on a review of two staff records, the center did not ensure all staff who worked directly with children obtained at least 16 annual hours of staff development activities that are related to child safety and development and the function of the center. Evidence: The licensing inspector observed the record for Staff #1 (hire date 2/26/15) contained documentation of completing seven training hours for the annual period beginning 2/26/18. Plan of Correction: The center will ensure that all staff who work directly with children will complete 16 hours of annual training.
