Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(2)-60-A (Children's records); 22VAC40-185-(2)-70-A (Staff records); 22VAC40-185-(4)-270-A (Building maintenance); 22VAC40-185-(4)-330 (Play areas); 22VAC40-185-(7)-500 (Hand washing)

Comments:
An unannounced monitoring inspection was conducted on 9/26/19 from 1:50pm - 4:00pm. During the inspection there were 74 children ages three months old through nine years old in care with 18 staff. Children were observed participating in various activities in the classrooms, eating snack, playing outside, and during rest period. Records were reviewed for seven children and seven staff during the inspection. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and interviews, it was determined that the facility did not ensure that all areas and equipment of the center shall be maintained in a safe and operable condition.

Evidence:
1. The bottom corner of the door in the School Age classroom that leads to the playground is deteriorated and has exposed sharp edges.
2. Staff #4 (Program Director) confirmed the door in the School Age classroom was not maintained in a safe and operable condition.

Plan of Correction: The facility responded: We will have our maintenance man repair or replace the door.

Standard #: 22VAC40-185-330-B
Description: Based on observation, it was determined that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

Evidence:
1. The resilient surface (poured-in-place), around the play structure on the School Age playground has several areas where there are holes in the surface so the proper depth is not being maintained.
2. Staff #4 (Program Director) viewed the resilient surfacing (poured-in-place) in the fall zone for the play structure, and confirmed that it was not compliant with the requirements in the standards.

Plan of Correction: The facility responded: Our maintenance man has been consulting with the manufacturer to determine the best way to repair the resilient surface. One a decision has been made, we will repair the resilient surface to eliminate the damaged areas.
