Violations:
Standard #: 22VAC40-185-510-G
Description: Based on review of medications and authorization forms, the program did not obtain a written authorization form for a long term medication found on-site.

Evidence:
The written medication authorization form, for a long term medication found on-site for child#1, were completed on a Loudoun County Public School medication authorization form, this form does not authorize the CASA program staff to administer the medication.

Plan of Correction: We have already talked to the parent, and we have requested a new authorization form.
