Comments:

The licensing inspector conducted an unannounced monitoring inspection on Monday, September 30, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at approximately 11:00am and concluded at 1:35pm. The center's assistant director was present and assisted the inspector throughout the inspection. The census for today's inspection consisted of 57 children in the direct care of 11 staff members. Upon the inspector's arrival, the children were engaged in a variety of activities. The children were observed during free play playing with puzzles, blocks, drawing, coloring, painting, and during classroom transitions. Some of the children were observed during teacher-led small group activities. Some of the children were observed eating lunch and others as they prepared to eat lunch. Hand-washing procedures were observed. Staff were observed having positive interactions with the children. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. The required postings and required practice drills were observed and found to be in compliance. The first aid kit and non-emergency supplies were inspected. Five children's records, five staff records. The center has CPR, First Aid, Daily Health Observation, and EMAT trained staff on site. The menu was posted and today's lunch consisted of black beans with rice, salsa, broccoli, mixed fruit, and milk. Evacuation drill: 09/27/2019 Shelter-in-place drill: 03/27/2019. One more shelter-in-place drill will have to be conducted for 2019. Standard #:	22VAC40-185-160-A
Description:	Based on a review of five staff records on 09/30/2019, the center did not ensure three staff submit documentation of a negative tuberculosis (TB) screening within the required time frame. Evidence: 1) The record for Staff #1, hired on 05/02/2019, contained documentation of a negative TB screening that was dated 07/25/2019. 2) The record for Staff #3, hired on 06/05/2019, contained documentation of a negative TB screening that was dated 07/03/2019. 3) The record for Staff #4, hired on 08/08/2019, contained documentation of a negative TB screening that was dated 09/10/2019. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering.
Plan of Correction:	Per the Center: Moving forward, staff will have 21 days to provide the documentation. If staff do not submit the documentation, they will be put on suspension until the results are received.
