Technical Assistance:
Technical assistance was provided regarding diaper cream authorizations, tight fitting crib sheets, and the playground area.

Comments:
An unannounced monitoring inspection was conducted today from 9:30am-1:15pm. There were 35 children directly supervised by 6 staff. The physical plant, outdoor playground area, programming, menus, 4 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating morning snack which consisted of snack mix and apple juice, listening to story time, learning about science experiments, playing outside, and having lunch which consisted of toasted cheese sandwiches, mixed fruit, cucumbers, and milk. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children. Standard #: 22VAC40-185-130-B
Description: Based on review of 5 children's records, 1 child's record did not contain documentation of additional immunizations once every six months for a child under the age of 2 years.

Evidence:
1. Child #4's (Age: 23 months) record has documentation that immunizations were last updated on 8/6/18.

Plan of Correction: I will request the parent to provide an updated immunization record.

Standard #: 22VAC40-185-240-D-5
Description: Based on staff record review and staff interview, there is not at least one staff member on duty who has obtained within the last 3 years instruction in performing the daily health observation of children.

Evidence:
1. The Director stated that there were no staff present at the center during inspection who have had daily health observation training within the last 3 years.

Plan of Correction: I will get the required training.

Standard #: 22VAC40-185-350-E-2
Description: Based on observation and record review, the center did not maintain a ratio of 1 staff to every 5 children for children ages 16 months old to two years.

Evidence:
1. At the time of inspection, there were 8 children present in the Duckling group outside on the playground with 1 staff member. The youngest child in the class is age 23 months and requires a ratio of 1 staff to 5 children.

Plan of Correction: I will plan to cover the need of our classrooms according to age and ratio requirements.

Standard #: 22VAC40-185-500-B
Description: Based on observation, a diaper changing pad is no longer nonabsorbent.

Evidence:
1. In the Sonshine Babies classroom, the diaper changing pad has multiple tears and is no longer nonabsorbent as the filling inside is exposed. The changing pad can no longer be properly cleaned and sanitized.

Plan of Correction: A new changing pad has been ordered and will be delivered tomorrow. In the meantime we will use a paper barrier on the changing pad.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of 4 staff records, 1 staff record did not contain documentation of central registry search results within 30 days of beginning employment.

Evidence:
1. Staff #2's (Date of Hire: 8/27/18) record did not contain documentation of the results of a central registry search.

Plan of Correction: I will have the staff complete the form again and we will get it sent out by tomorrow.
