Comments:
An unannounced renewal inspection was conducted today from 10:30am-1:30pm. There were 37 children (18 months-6 yrs.) supervised by 8 direct care staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed tracing, practicing cursive writing, reading, playing a card matching game, painting, and playing outdoors. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, 4 of 5 children's medications did not contain documentation of with written authorization from the parent.

Evidence: Child #3's, Child #2's, and Child #4's medication authorization forms did not contain documentation of parent signatures.

Plan of Correction: We are sending forms home to get parent signatures today.

Standard #: 22VAC40-185-550-D
Description: Based on record review, the center did not maintain documentation of a monthly practice of evacuation drills.

Evidence: There was no documentation of a fire drill completed for May 2018.

Plan of Correction: We will be sure to document all drills.
