Comments:
Thank you for the time and assistance with the unannounced focused monitoring inspection that was conducted on September 24, 2019 from 2:55 p.m. to 3:45 p.m. The inspection was a follow-up to violations cited during a complaint inspection which began on April 23, 2019. There were 76 children present in seven classrooms and in the care of 12 staff members with three additional staff members available to assist if necessary. One staff member was interviewed and the medications and mediation paperwork were reviewed for three children. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page.

Violations:
Standard #: 22VAC40-185-510-C
Description: Based on review of three medications and the corresponding paperwork, there was a long-term medication on-site with an expired physician's authorization.

Evidence:
1. The written physician's authorization allowing the center to administer medication to Child C was valid from 07/09/2019 through 09/03/2019. The authorization had expired and the medication was on-site.
2. Staff Member A confirmed the physician's authorization had expired.

Plan of Correction: The staff will speak with the parent of the child as soon as possible and ensure that either the medication is taken home or an updated physician's authorization form is obtained.
