Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(2)-60-A (Children's records); 22VAC40-185-(2)-70-A (Staff records); 22VAC40-185-(4)-260-A (Annual Fire inspection); 22VAC40-185-(7)-510- (Medication)

Comments:
An unannounced monitoring inspection was conducted on 9/30/19 from 9:20am - 11:30am. During the inspection there were 53 children ages two years old through five years old in care with 13 staff. Children were observed participating in various activities in the classrooms, and playing outside. Records were reviewed for five children and six staff during the inspection. Medication, emergency procedures and emergency supplies were reviewed during the inspection. There were no violations cited during this inspection.

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on a review of documentation and interview, it was determined that the facility did not ensure that an annual inspection report from the appropriate fire official having jurisdiction was completed.

Evidence:
1. The most recent annual fire inspection report inspection available for viewing during the inspection was dated 9/28/17.
2. Staff #6 (Program Director) confirmed that the annual fire inspection report had not been completed.

Plan of Correction: The facility responded: The fire marshal was contacted the same day, and an inspection was scheduled for for 10/4/19.

Standard #: 22VAC40-185-280-B
Description: Based on observation, it was determined that the facility did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. The door for the closet in the classroom #123 was open, and the closet contained several bottles of hazardous substances used for cleaning.
2. There were two containers of Lysol cleaning wipes in an unlocked cabinet over the sink in classroom #128.
3. Staff #6 (Program Director) confirmed there were hazardous substances present that were not kept in a locked place.

Plan of Correction: The facility responded: The closet door in classroom # 123 was secured and locked during the inspection. The cleaning wipes in classroom #128 were moved to a locked cabinet. Teachers will be re-trained about storage of any hazardous substances in a storage area that can be locked.
