Technical Assistance:
A discussion was held on getting parental permission for the medical authorization form.

Comments:
Today an unannounced renewal study was conducted between 8:15am & 9:30am. There were 31 children present with 4 staff. Children were observed arriving, playing outside, playing games, and dismissal for school. Physical plant, programs, children's records, medications, and emergency drills were also reviewed. Positive interaction between children and staff was observed. Staff records are reviewed annually in the administrative office, however, MAT (medication administration training) and first aid/CPR certificates were available for review. Description: Based on review of 3 medication authorizations, 2 do not have parental permission.
Evidence:
Child #1 has 2 medications on site without parent authorization to administer.

Plan of Correction: Parent refuses to sign medication authorization forms. I will discuss with my supervisor.
