An unannounced investigation was conducted 09/13/2019 in response to a self-reported incident regarding the care of children. During the investigation, staff were interviewed, parents were interviewed, staff files were reviewed, children's files were reviewed and the toddlers room was observed. The preponderance of evidence does not support the allegation; therefore, no violations were cited related to the self-reported incident.The exit interview was conducted with the quality assurance director.
