Technical Assistance:
The following was discussed with administration during the inspection:
40-185-290 Guardrails US BC

Comments:
An unannounced monitoring inspection was conducted between 9:00am and 11:00am. Upon arrival there were 49 children in care with 7 staff present. A sample of 3 staff and 5 children's records were reviewed in addition to 1 medication. Children were observed during morning snack, in circle time and being read a story.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure all areas and equipment of the center, inside and outside, shall be maintained in a safe condition.
Evidence:
1. There was peeling paint and rusted hardware on the merry-go-round.
2. The wooden railing leading to the playground had areas that were splintered and in reach of the children.
3. The bottom of the wooden door frame, entering the center from the playground, was rotted in the right corner.

Plan of Correction: The merry-go-round did have some repair areas that were missed in the original playground safety
check prior to school starting. If the areas noted per violation notice, on this equipment, cannot be fixed
to be free of all rust, the equipment will be removed to ensure playground environment remains safe
per social service standards. The wooden door frame to the outside door leading out to the playground will need to be replaced. Therefore, our contractor ?Under Construction? has been contacted on September 10, 2019 to set a date to get this area of the playground (door frame and railings) free of any rotten wood or potential
splintering. All work will be completed as soon as possible. Bullfrogs and Butterflies overhauls the playground each new school year. This school year, new mulch was added, 11 fence panels replaced, sanding and painting the metal playground equipment, and all plastic houses/hoops were cleaned with anti-mold cleaner prior to September 3, 2019. As a plan of correction, we will continue to do our best in monitoring the playground to ensure the environment is
safe and free from all hazards.

Standard #: 22VAC40-185-280-G
Description: Based on observation the center did not ensure substitute containers were clearly labeled indicating its contents.
Evidence:
There was a spray bottle in the kitchen area that contained a liquid substance and the bottle was not labeled indicating it's contents. The staff stated the bottle contained soap and water.

Plan of Correction: The unlabeled spray bottle in the kitchen area kept on a high shelf, out of reach of children contained
soap and water to clean the kitchen tables.This violation notice provided our facility information that our process to wash the tables with soap and water, and spray after with Lysol (EPA approved) is now an incorrect cleaning process.Therefore, our plan of correction is to discontinue the use of soap/water and Lysol and we will start to
use a Cintas labeled bottle and hard surface cleaner that is EPA approved as of September 13, 2019. This
cleaner will be locked up instead of being kept out of reach of children.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on observation the center did not ensure all staff had the required background checks.
Evidence:
1. Staff #3 indicated living outside of the state of Virginia in the past 5 years. Staff #3 (hired 02/09/19) did not have documentation of an out-of-state central registry check.

Plan of Correction: All background checks; both finger printing and central registry check were completed for staff #3 in the
state of Virginia. The background check for out of state (RI) was mailed on 9/10/2019. New ruling to
have out of state background checks for any employee living out of state within the last 5 years will be
closely monitored and completed for all new hires going forward.
