Comments:
The licensing inspector conducted an unannounced renewal inspection on Wednesday, September 11, 2019 between 12:00 pm to 1:30 pm determine the center's compliance with licensing standards. During this inspection, the children in care were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 36 children with seven staff present including volunteers. The licensing inspector completed observations of the children in care completing a variety of activities to include eating lunch, transitioning from lunch, completing bathroom breaks/handwashing,engaged in interactive song, reading books on carpet, and preparing for napping, and napping. The children's lunch today consisted of tuna salad, wheat crackers, cucumbers with dip, pineapple tidbits, and milk. The program's classrooms, bathrooms, designated childcare areas were inspected by the licensing inspector. The required posting were reviewed by the inspector. There were six children?s records reviewed during this inspection. The staff present emergency contact and first aid/CPR certificate of completion, MAT and training information were reviewed by licensing inspector. The center's first aid kit and emergency supplies were inspected. Medication is being administered, medications, and written authorizations were reviewed for the center's compliance. The Department has received a complete renewal application and annual fee from the center at this time. Description: Based on review of children's record and interview with staff, the center did not ensure to maintain a record that contain all of the following information.

Evidence: In Child#3's record (start date 9/03/19), the address for the second designated emergency contact was incomplete.

Plan of Correction: Teacher will contact case manager and get completed address for following student's record.
