Technical Assistance:
New Regulations discussed. Proof of Identity is a requirement for all records.

Comments:
An unannounced Subsidy Inspection was conducted from 950-1050 AM. The Vendor was present with 2 children ( one lives in household) for a total of 5 points. The children receive care in the main room and Vendor stated bedrooms are off limits. The children had toys and materials available and children were observed playing and drinking juice. Records were reviewed and attendance record keeping is current. Vendor has conducted and recorded monthly drills and the annual lockdown drill. Vendor's CPR and First aid certification will expire in June but she stated she is scheduled for training on June 1, 2019. Please call with questions 703-479-4679

Description: Based on review of a child's record, it was determined that all information was not maintained by the vendor. Evidence; 1. Child #1 's file was missing parent's work phone number and plan of action for a food allergy. Straberries was listed as an allergy but no documentation on what to do in the event the child has an allergic reaction.

Plan of Correction: I will have parent fill out paperwork and give me a phone number.
