Comments:
An unannounced Monitoring inspection was conducted on October 2, 2019. The Licensing Inspector arrived in the center at 8:45 AM and departed the program at 11:30 A.M. There were 9 children and 2 staff present at the time of the inspection. The children were observed in their classroom and in the outsider. Staff were observed interacting with the children. Observed activities included: individual-selected center play, hand-washing procedures, snack,and circle time.
The inspector reviewed five children's records. Documentation of fire and health inspections was reviewed.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, the center failed to ensure that all physicals examination were signed by the physician or designee.

Evidence:
1. Child # 1 had a physical exam that was printed from My Chart. The physical was not signed by the physician or designee.

Plan of Correction: The FSW and staff will have the parent to obtain a new physical exam that is signed.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, the center failed to ensure that all records were complete as required by the standard.

Evidence:
1. Child # 3 did not have employment information for both parents.
2. Child # 5 did not have a phone number for one of the two required emergency contacts.

Plan of Correction: The FSW will obtain the missing information and will monitor all records.

Standard #: 22VAC40-185-280-E
Description: Based on observation, the center failed to ensure that cleaning materials (e.g., detergents, sanitizers and polishes) and insecticides/pesticides were stored in areas physically separate from each other.

Evidence:
1. A spray can of pesticide was stored under the stink with the center's cleaning supplies.

Plan of Correction: The lead teacher removed the spray and will return the can to the maintenance man.
