Technical Assistance:
Technical assistance was offered regarding background checks, children's records,

Comments:
An unannounced monitoring inspection was conducted from 9:30 am-12:00 pm. 26 children were in care with 7 staff members present. Children were observed during snack time, engaged in group activities and outdoor play time. 4 staff and 5 children's records, medications and other documents were reviewed. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not maintain complete records for children enrolled.
Evidence: 1. Child # 1's record was missing the following items: Parent's name, address, work information, information for emergency contacts and persons authorized to pick up, first day of attendance, parent agreement and proof of identification.

Plan of Correction: I will get the missing information and maintain a complete record.

Standard #: 22VAC40-185-70-A
Description: Based on record review, the center did not obtain two references for an employee before the first day of employment.
Evidence: Staff # 3 was hired on 9/10/19. There was documentation of only one reference check in Staff # 3's record.

Plan of Correction: The reference check will be obtained, documented and kept on file.

Standard #: 22VAC40-185-270-A
Description: Based on observations, areas outside the center were not maintained in safe and clean condition.
Evidence:
1. On the playground, a large broken red piece of equipment was found laying on the ground.
2. A white trash bag was found sitting in the corner of the playground.
3. The wooden platform that was in children's use had corner that was broken and splintering.

Plan of Correction: The broken piece and trash bag will be removed, and repairs will be made to the platform.

Standard #: 22VAC40-185-340-D
Description: Based on record review and staff interview, a program leader supervising more than two aides in a group of children.
Evidence:
1. There were two groups of two year old children at the center.
2. Staff # 1 leading the two groups with three assistant teachers.

Plan of Correction: I will have one of the assistants get lead qualified.

Standard #: 22VAC40-191-60-B
Description: Based on record review, the center did not obtain a Sworn Disclosure Statement (SDS) for an employee before the first day of employment.
Evidence: Staff # 2 was hired on 9/2/19. An SDS was not obtained for Staff # 2 before the first day of employment.

Plan of Correction: I will have the staff person complete the form and place in her record.
