Technical Assistance:
Technical assistance was offered regarding children's records.

Comments:
An unannounced monitoring inspection was conducted from 4:20 pm-5:30 pm. 18 children were in care with 2 staff members present. Children were observed eating afternoon snack, playing with tablets and other floor toys. Medications and evacuation drill records were reviewed. 1 staff record and 5 children record were reviewed. Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not maintain complete records for children enrolled.
Evidence: Child # 2's record was missing the following information: information for second parent, emergency contacts and persons authorized to pick up.

Plan of Correction: I will have the parent complete the form.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were stored in a locked cabinet, making them accessible to children.
Evidence: The teacher's cabinet had a large bottle of bleach and two bottles of disinfectant sprays. The cabinet was unlocked making these items accessible to children.

Plan of Correction: The cabinet was locked during the inspection.

Standard #: 22VAC40-185-510-G
Description: Based on observation, the center did not obtain written parental authorization for medications on site.
Evidence: There were two emergency medications on site for Child # 1. The center did not obtain written parental or physician's authorization for both medications.

Plan of Correction: We will ask the parents to bring in the authorizations.

Standard #: 22VAC40-185-550-D
Description: Based on record review and staff interview, the center did not practice monthly evacuation drills.
Evidence: The evacuation drill did not have any documentation of an evacuation drill for the month of September 2019. Staff # 1 stated that a drill was not practiced.

Plan of Correction: Evacuation drills will be practiced monthly and recorded.
