An unannounced monitoring inspection was completed on 10/03/2019. There were 7 children, ages 3 years-4 years, 2 staff providing direct care and supervision and additional support staff were present during the inspection. The inspector reviewed 5 children's records, 2 staff records and 1 medication during the inspection. Children were observed in the following activities: in large motor play in the gym, during circle time, having a morning snack, going outside to play and during free choice activities. The inspector reviewed the emergency supplies during the inspection. The inspector discussed transportation and the resources available on the public website with the staff. Standard #: 22VAC40-185-60-A Description: Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements required by the standards.
Evidence:
1. The record for Child #1 did not contain information on allergies and intolerance to food, medication, or other substances, and actions to take in an emergency situation.
2. The record for Child #2 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; there were not address provided for the designated people listed in the record.
3. The record for Child #3 did not contain information on allergies and intolerance to food, medication, or other substances, and actions to take in an emergency situation.

Standard #: 22VAC40-185-240-D-5 Description: Based on record review and discussion with the staff, the facility failed to ensure that there shall always be at least one staff member of duty who has obtained within the last three years instruction in performing the daily health observation of children.
