Comments:
An unannounced Monitoring Inspection was conducted on October 2, 2019 from approximately 8:45 am until 10:30 am. Upon entrance there were 35 children and 5 staff present with children. The record review sample consisted of 5 children's records and the emergency information and health related information for 5 staff records. Three medications were reviewed. Children were observed during their morning routine, to include reading, eating breakfast, singing and dancing. The results of the inspection were reviewed and verified with the Center Lead Teacher. during the exit interview. Areas of noncompliance are noted on the violation notice. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and staff interviews, the licensee did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence: On the counter near the sink, in rooms #525 and #524 there was a microwave oven sitting on top of a plastic crate. The microwave was larger in length and depth than the crate. The microwave was not secure as it easily moved when touched. The microwave measured between 18 and 19 inches in length and between 12 and 13 inches deep and the crate measured between 12 and 13 inches in length and 10 to 11 inches deep. The microwave was accessible to children in room 524 and 525 and posed a tipping hazard.

Plan of Correction: The center staff removed the crates that were placed under the microwave during the inspection.
