A Subsidy Health and Safety inspection was conducted on 09/30/2019 to review supplemental health and safety requirements.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: 22VAC40-665-580-B Description: Based on review of staff files, documents, and staff statements the vendor failed to maintain compliance with this standard as required related to the Preservice Training.
Evidence:
Staff #2, hire date 7/5/19, and Staff #3, hire date 4/4/19, did not have documentation of completion of the 10 hour Preservice Training. The allowed 90 days to complete this training has lapsed.Plan of Correction: These staff will complete this training immediately and the proof of completion document will be placed in their file for compliance. Verification of completion/status of this required training will be followed up on weekly by administration for any new staff to ensure future compliance.
Standard #: 22VAC40-665-580-E-1 Description: Based on review of staff files, documents, and staff statements the vendor failed to maintain compliance with this standard as required related to CPR.
Evidence:
Staff #3, hire date 4/4/19, did not have CPR as require; the allowed 90 days to obtain this had lapsed.
Staff #1 and #4 did not have current CPR as required; their CPR had expired as of August 2019.
Staff #5 did not have current CPR as required as it had expired as of April 2019. Staff did do an on-line only CPR course, however this type of on-line only course is not allowed and does not count for compliance purposes.Plan of Correction: A CPR and FA course will be scheduled for staff so that compliance will be met. In the future compliance with this standard will be followed.
Standard #: 22VAC40-665-580-E-2 Description: Based on review of staff files, documents, and staff statements the vendor failed to maintain compliance with this standard as required related to first aid.
Evidence:

Staff #1 and #4 did not have current FA as required; their FA had expired as of August 2019.
Staff #5 did not have current FA as required as it had expired as of April 2019.
