Technical Assistance:
Discussed diapering trashcans, parents' place of employment, and annual training hours.

Comments:
An unannounced renewal inspection was conducted today from 10:10am-1:45pm. There were 91 children (infants-5yrs.) supervised by 17 direct care staff. The physical plant, 5 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed going on a buggy ride, painting, playing with playdough, puzzles, and animals, having storytime, and eating snack. Snack served was strawberries, animal cookies, and water. Toileting and hand-washing procedures were also observed. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-60-A
Description: Based on record review, 10 of 10 children's records did not contain documentation of all required information.

Evidence:
1. Child #10's record did not contain documentation of either parent's place of employment.
2. Child #2, 4, 5, 6, 7, 8, and 10's records did not contain documentation of children's first day of attendance.
3. Child 1-10's records did not contain documentation of previous child day care and schools attended by the child.

Plan of Correction: I will update records and create a form for parents to document children's attendance at other schools. It will be added to children's records by 10/31/2019.

Standard #: 22VAC40-185-270-A
Description: Based off observation, 1 of 8 classrooms did not ensure that all areas and equipment of the center were maintained in a clean, safe, and operable condition.

Evidence: In the Apples classroom (ages 12 months-2 years), a drawer by the classroom sink had a broken child safety lock.

Plan of Correction: We will replace the lock today.
