A Subsidy Health and Safety inspection was conducted on 09/27/2019 to review supplemental health and safety requirements.
The subsidy requirements were reviewed this date: allergy action plans, staff training, nutrition and food services, record keeping, hazardous substances, reporting injuries and emergency plans.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-530-4 Description: Based on record review, not all staff have certifications of first aid and cardiopulmonary resuscitation.
Evidence:
1) Staff #1's record did not have documentation of having completed CPR or first aid training.Plan of Correction: We will schedule and attend classes within 30 days.
Standard #: 22VAC40-665-580-B Description: Based on record review, not all staff who work directly with children had completed the Virginia Preservice Training for Child Care Staff within the first 90 days of employment and the Recognizing Child Abuse and Neglect and Reporting Responsibilities Training.
Evidence:
1) Staff #1 (start date 03/2019), Staff #2 (start date 10/03/2018) and Staff #3's (start date 11/2015) records did not have documentation of having completed the Preservice Training.Plan of Correction: Staff will complete the training within 2 weeks.
