Comments:
An unannounced renewal inspection was conducted between the hours of 9:30 am-12:30pm. At the time of entrance, 55 children were in care with 15 staff members present. Children were observed participating in a science project, creating art work, reciting letters, participating in free play, and performing yoga. The children were actively engaged in activities and interacted positively with staff. A selection of 5 staff and 5 children records, the physical space, emergency supplies, evacuation drills, and attendance records were reviewed. Standard #: 22VAC40-185-60-A
Description: Based on review, the record for each child enrolled did not contain all required information.
Evidence:
1. The records for Children 1-5 did not contain documentation of home address for each parent who has custody.
2. The record for Child #3 did not contain confirmation that the required information in the child's record is up to date annually.
3. The most recent update in the record for Child #3 signed by a parent is dated 08/06/2018

Plan of Correction: I will add an addendum to the records to update the information and update the form moving forward.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In Classroom A3, disinfectant wipes were observed on the top shelf of an unlocked closet.
2. In Classroom A4, disinfectant wipes and spray were observed on the top shelf in an unlocked closet. Upon inspection, door was locked by teacher.

Plan of Correction: Corrected during inspection.
