Comments:
The licensing inspector conducted an unannounced monitoring inspection on Wednesday, September 18, 2019 between 10:00 am until 12:31 pm determine the center's compliance with licensing standards. During this inspection, the children in care were observed by the licensing inspector in the two classroom settings and at the outdoor area. The census for today's inspection consisted of 29 children with five staff present. The licensing inspector completed observations of the children in care engaged in active outdoors, transitioning indoors, participating in circle time, working in center, interacting with peers, completing bathroom breaks, and preparing for napping. The center's bathrooms, and designated classrooms, and equipment were inspected by the licensing inspector. The required posting were reviewed by the inspector. There were five children?s records reviewed by the inspector during the inspection. The center's first aid kit and emergency supplies were inspected. There were five staff records and available emergency contacts for staff reviewed during this inspection. Standard #: 22VAC40-185-130-A
Description: Based on review of children's record and interview with staff, the center did not ensure to obtain documentation of immunization before a child can attend.

Evidence: In Child#1's record (start date: 09/03/19), there was no documentation of an immunization found. Interview with staff confirmed the documentation for immunization of the child was not available during the inspection.

Plan of Correction: Contact parent to request a copy, child can't return until received.

Standard #: 22VAC40-185-70-A
Description: Based on interview with staff, the center did not ensure to maintain staff emergency contact information at the site for review.

Evidence: Staff#4 and Staff#5 were missing emergency contact documentation at the center during the inspection. Interview with staff confirmed the documentation was not available for review.

Plan of Correction: The program will ensure all staff contact is on site.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In Classroom 2, there was cleaning equipment with the labels "keep out of reach of children" and "warning" located on the floor accessible to children in care.
2. There were three large containers of flooring products with the labels "keep out of reach of children" and "warning" outside of the classroom areas which were accessible to children in care.

Plan of Correction: Corrected during the inspection, The program will ensure all hazardous substances will be locked. Other hazardous will be placed in outdoor classroom #3.

Description: Based on review of emergency supplies, the center did not ensure the first aid kit included all of the required items.

Evidence:The center's first aid kit was missing an ice pack or cooling agent and triangular bandages.

Plan of Correction: The program will purchase batteries and other missing materials in the first aid kit.

Standard #: 22VAC40-185-540-E
Description: Based on review of emergency supplies and interview with staff, the center did not ensure to maintain all required supplies at the program.

Evidence: During the inspection,there was not a working battery operated radio. Interview with staff confirmed the battery operated radio was not working due to the need for extra batteries.

Plan of Correction: The program will purchase batteries and other missing materials in first aid kit.
