Health and Safety Update is to be completed by staff no later than 3/1/2020.

A Subsidy Health and Safety inspection was conducted on 9/17/2019 to review supplemental health and safety requirements. Reviewed physical space, records, training certifications and allergy requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-540-A Description: Based on record documentation, staff #2's TB screening is not within the last 30 days of calendar hire. (doh 6/24/2019).
Evidence: TB screening documentation was dated 6/19/2018 and exceeded the thirty calendar days allowed for an acceptable TB documentation.Plan of Correction: Staff member #2 redid the TB test and has submitted documentation to be placed in her personnel file.
