A non-mandated 60 day monitoring inspection during the Conditional Licensing period was conducted today. There were 63 children present with 10 staff. Children were observed having circle time, reading books, playing with manipulative toys and napping. Reviewed 6 staff files, 5 children's files, medications and authorizations, emergency evacuation drills, attendance, menus, and physical plant. Standard #: 22VAC40-185-240-D-4 Description: Based on observation and interview with staff, a child's emergency medications were not in the care of a staff member.
Evidence:
Child#7's Epi pen was left outside on the playground while child was inside. Staff stated they forgot it and had just come in from the playground.Plan of Correction: This violation was corrected at the time of inspection by staff retrieving the medication and bringing it into the classroom where the child was housed.
Standard #: 22VAC40-185-440-E Description: Based on observation, there was not at least 12 inches of space between occupied cots, beds, and rest mats.
Evidence:
In Montessori 2 classroom, cots were observed spaced 3-4 inches apart.Plan of Correction: This violation was corrected at the time of inspection by the Director moving the cots and spacing them 12 inches apart.
Standard #: 22VAC40-185-510-A Description: Based on observation, center did not have written authorization to administer medication to a child.
Evidence:
Child#6's medication authorization listed another child care facility's name as having permission to administer. Stone Ridge Montessori was not listed on the form and therefore did not have permission to administer.Plan of Correction: Director call parent during this inspection to get permission slip. Parent stated they will bring the proper form in this week.
