Comments:
The licensing inspector conducted an unannounced monitoring inspection on Wednesday, October 2, 2019 between 9:40 pm to 11:00 am to determine the center's compliance with licensing standards. During this inspection, the children in care were observed by the licensing inspector in the classroom settings and at the outdoor area. The census for today's inspection consisted of fifteen children with three staff present. The licensing inspector completed observations of the children in care engaged in active outdoor play, transitioning indoors, participating in center enter, interacting with peers and staff, and completing bathroom breaks. The center's bathrooms, and designated classroom, and equipment were inspected by the licensing inspector. The required posting were reviewed by the inspector. There were five children?s records reviewed by the inspector during the inspection. Medication is being administered, medications, and written authorizations were reviewed for the center's compliance. The center's first aid kit and emergency supplies were inspected. Description: Based on review of children's record and interview with staff, the center did not ensure records contain all information.

Evidence:In Child#1's record (start date 09/03/19) , the written agreement between the parent and the center as required was not found. Interview with staff confirmed the written agreement was not available during the inspection.

Plan of Correction: The staff will contact case manager to obtain document to ensure children records are complete.

Standard #: 22VAC40-185-260-A
Description: Based on review of fire inspection and interview with staff, the center did not ensure an annual fire inspection report was complete.

Evidence: The last documented fire inspection at the program was dated 03/30/18. Interview with staff confirmed an updated fire inspection report was not available for review during the inspection.

Plan of Correction: The staff will contact program contact person to obtain updated copy of fire inspection.
