An unannounced investigation was begun in response to a self reported incident.
On August 4, 2019 an infant was fed a bottle of breast milk belonging to another child.

Standard #: 22VAC40-185-340-A Description: Based on multiple staff statements, and a review of documentation, the center did not ensure care, protection and guidance of one infant in care.
Evidence:
On 8/5/19 Staff A failed to verify the name on the baby bottle matched the child to which it was being fed. Staff A gave Child #1 a bottle containing breast milk that belonged to another child. (Child #2).Plan of Correction: We took that teacher out of working with children that have bottles (Toddlers and below). All staff that work with infants and are fed bottles were given infant feeding training. All staff signed an acknowledgment of training.
