As substitutes or new staff assume a role, please ensure that training documentation is maintained at the center.
Discussed the need to maintain ratios at all times and provisions for situations that would prevent ratios to be maintained.
An unannounced Monitoring inspection was conducted this date. A total of 31 children with 2 staff present. Transition from the classroom setting to the programming area was observed. The children were observed arriving and engaging in table top activities such as playdoh, drawing, playing in small groups with cars and magnets and legos. Books available that were diversified for the age groups present. Hand washing and snack time was prepared. One child on 3 medications. All 3 medications were reviewed and found complete. A total of 6 children's records were reviewed. CPR/First Aid, MAT and Daily Health Observation training was verified. Staff records are reviewed to include all background checks on an annual basis at the administrative office. This inspection began at approximately 2pm and ended at 4:35pm. Standard #: 22VAC40-185-70-A Description: Based on staff record review, one record did not include all required information.
Evidence: Staff #2 was present during the inspection. No documentation of an emergency contact was found for this staff.
Plan of Correction: I will maintain an emergency contact on site.
Standard #: 22VAC40-185-350-E-5 Description: Based on observation and staff statements, child to staff ratios were not maintained.
Evidence: The Licensing Inspector observed Staff #1 in the programming room with 29 children for approximately 4 minutes. The Licensing Inspector asked Staff #1 where Staff #2 was. Staff #1 stated to the Licensing Inspector, "I'm not sure where [they] are." During this time, Staff #2 observed Child #1 running out of the programming room. Staff #2 left the programming room to follow Child #1, leaving Staff #1 with 29 children.
Plan of Correction: We will discuss an emergency plan to address when children attempt or leave the cafeteria or gym that will ensure that ratios are maintained at all times.
Standard #: 22VAC40-185-560-F Description: Based on observation of the programming area and staff statements, a current menu snackwas not posted and is not provided to parents.
Evidence: The menu posted during the inspection if for the month of September.
Plan of Correction: I will print out October snack menu and will post it.
