63.2(17) License & Registration Procedures
- Technical Assistance:

Have six months of supervised programmatic experience.
a. Within six months before being promoted or beginning work or one month after being promoted or beginning work, a minimum of 12 hours of training shall be received related to the care of children, including but not limited to:
(1) Child development;
(2) Playground safety;
(3) Health and safety issues; and
(4) Preventing and reporting child abuse and neglect.
b. Such training may take place on site while not supervising children.

An unannounced Monitoring inspection was conducted this date. A total of 41 children with 6 staff were on site today. The children were taking afternoon naps when the Inspector arrived. Walk thru of all occupied classrooms was made. Staff interviews conducted. A review of 5 Staff records and 4 children's records was made. Previous violations were reviewed from the April inspection, two repeated issues found today. This inspection began at approximately 12:30pm and ended at 3:15pm. Thank you for your cooperation today. Standard #: 22VAC40-185-70-A Description: Based on staff record review, documentation of reference checks were not found.
Evidence: Staff #3 and #4's records do not contain documented completion of 2 references.Plan of Correction: We will ensure that all new staff have reference checks completed prior to hire.
Standard #: 22VAC40-185-210-A-4-a Description: Based on staff record review, 2 staff are in lead teacher roles who do not meet the lead teacher qualifications.
Evidence: Staff #1 and #2 have been in lead teacher roles for over one month and have not received the 24 hours of lead teacher qualifications to include child development, playground safety, child abuse and neglect and health and safety.Plan of Correction: These two teachers have bachelor's degrees but are not in education. We will provide this training to the staff by the end of this month.
Standard #: 22VAC40-185-510-G Description: Based on review of medications on site, one medication authorization did not include the Center to administer.
Evidence: Authorization for an emergency medication for Child #5 was given to a public school entity and not for the Center. The authorization specified the public school had authorization to administer and did not include the Center on the signed authorization.Plan of Correction: We will have the physician complete the state form or MAT form for authorization for this medication.
Standard #: 22VAC40-191-60-C-2 Description: Based on staff record review, a background check was not obtained for one staff.
Evidence: Staff #1 was hired on 2/8/19. A central registry finding was not on file to date.Plan of Correction: We have contacted office of background investigation and found out the email the results were sent to the wrong school. We will receive the results within 24 to 48 hours. Documentation of the background result will be forwarded to the Licensing Inspector within 10 days.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on staff record review, background check results were not obtained prior to hire.
Evidence: Staff #3 was hired on 10/1/19, fingerprint results were dated 10/4/19. Staff #4 was hired on 09/30/19, fingerprint results were dated 10/1/19.Plan of Correction: We will develop a system that no staff begin work without having the fingerprint criminal search result on file.
