Licensing Inspector arrived at 9:45 and departed at 11:00 AM to conduct a renewal inspection. At the time of the inspection, there were three preschool children in care with two staff. Children were observed watching television and playing with table top toys. The sample size was two staff and three children's records.

Standard #: 22VAC40-185-340-C Description: Based on interviews with staff, it was determined that the facility did not ensure that when one or more children children are present, there are at least two staff members on site.
Evidence:
Staff #1 stated that she arrives daily at 7:00 AM and staff #2 arrives about 8:00 AM. She stated that she usually has two or three children in care prior to staff #2 arriving.Plan of Correction: Staff #1 will modify the staff schedule so that staff #2 arrives prior to any of the children.
Standard #: 22VAC40-185-580-L Description: Based on interview and review of children's records, it was determined that the facility did not secure permission for transportation before the activity.
Evidence:
1. Staff #1 stated that she daily takes the preschool children with her when she goes on bus runs before and after school.
2. None of the children's records contained signed parental permission for the children to be transported on these van runs.Plan of Correction: Center director will type up a permission slip for transportation and have the parents sign them.
