Comments:
An unannouced renewal inspection was completed on 10/8/19. There were 92 children present in eight groupings with 19 direct care staff present, one kitchen staff, and two administrative staff present. Ten children's records and ten staff records were reviewed. Two emergency and two as needed medications were reviewed.

The inspector arrived at the center at 8:45 am and departed at 3:15 pm.

The children were observed during arrivals, during outside times, during lunch, and during nap. All infant activities were observed.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, the center failed to ensure that all information was documented as required for staff records.

Evidence:
1. There was no documentation that two or more references as to character and reputation as well as competency were checked before employment for staff #6. Staff #6 hire date was 9/11/19 and started working 10/2/19. There was no documentation that references were checked for staff #6.

2. The references for staff #2 were not completed before employment. Staff #2 was hired 9/12/19 and the references were dated 9/13/19 and 9/14/19.

Plan of Correction: The director will obtain references for staff #6. References will be completed before employment for all future hired staff.

Standard #: 22VAC40-185-370-1
Description: Based on observation, the center failed to ensure that sleeping infants shall be individually checked every 15-20 minutes.

Evidence:
1. There were three infants observed sleeping longer than 20 minutes and were not individually checked.

Plan of Correction: Staff will be reminded to individually check the infants while they are sleeping.

Description: REPEAT VIOLATION

Based on observation, the center failed to ensure that hand washing was practiced as required.

Evidence:
1. An infant room was observed. A staff person was observed to make a bottle for one infant after preparing and feeding a bottle for a different infant. The staff person did not wash her hands prior to making the bottle for a different infant after making a bottle, holding an infant and feeding an infant.

2. A staff person was observed to hand a bottle to another staff person to feed an infant. This staff person who was given the bottle was observed playing with several different infants and toys on the floor prior to this feeding. The staff person who fed the infant did not wash her hands prior to being given the bottle to feed the infant.

Plan of Correction: Staff will be reminded to practice hand washing as required.

Description: Based on observation and interview, the center failed to ensure that the diaper changing surface was cleaned and sanitized as required.

Evidence:
1. A diaper change was observed. The surface was cleaned with soap and water after the change. The diaper surface was not observed to be sanitized.

2. The staff person who had changed the diaper and cleaned the surface was asked if that was the only step that is practiced after a diaper change and told the inspector that "yes" it was the only step.

Plan of Correction: The diaper changing surface will be cleaned and sanitized as required after each diaper change.

Standard #: 22VAC40-185-520-C
Description: Based on observation, the center failed to ensure that all diaper ointments were labeled as required.

Evidence:
1. There was a diaper ointment observed in a cabinet with the diapers that was not labeled.

Plan of Correction: This was corrected during the inspection.

Standard #: 22VAC40-185-570-F
Description: Based on observation, the center failed to ensure that heated formula and baby food shall be stirred or shaken and tested for temperature before serving to children.

Evidence:
1. Two bottles at separate times during the inspection were observed to be heated in a crock pot of hot water. The bottles were not observed to be shaken or tested for temperature before being fed to two different infants.

Plan of Correction: Staff will be reminded to shake and test bottles before being fed to the infants.
