Description: Based on observation of the small playground and measurements, it was determined that the center did not maintain the required amount of resilient surfacing for all playground equipment. 1. Wood mulch measured 2 inches in depth in front of the chute of Slide #1.
2. Wood mulch measured 2.5 inches in depth on the left of Slide #1.
3. Wood mulch measured 3 inches in depth on the right side of Slide #2.
4. Wood mulch measured 3 inches in front of the chute of Slide #2.

Plan of Correction: In the future all staff will have fingerprint results on file before hire.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of staff records, the center did not request and follow up on out of state central registry background checks for all staff that resided outside of Virginia in the past five years as required.
