An unannounced renewal inspection was conducted between the hours of 9:30 am-11:20 am. 10 children were in care with 4 staff members present. Children were observed during snack time, and engaged in group activities. A selection of 4 staff and 5 children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. A subsidy health and safety inspection (SHSI) was also conducted today. Description: Based on record review, 1 of 5 children did not have a physical examination by or under the direction of a physician within one month after attendance.
The record for Child #2 (start date 09/03/19) did not contain documentation of a physical exam.

Plan of Correction: I will email the parent and request the physical.

Standard #: 22VAC40-185-160-C
Description: Based on record review, 1 of 4 staff did not obtain and submit the results of a follow-up tuberculosis screening every 2 years.
The most recent tuberculosis test results available for review in the record for Staff #2 was dated 8/23/2017.

Plan of Correction: Staff will complete the TB test by the end of the week.

Description: Based on record review and staff interview, injury prevention procedures were not updated at least annually.
1. The last documented review of the center's injury prevention plan was dated 08/15/2017.
2. Staff stated the injury prevention plan had not been updated since 08/15/2017.

Plan of Correction: I will review the injury prevention plan annually.

Description: Based on record review, 3 of 4 staff who work directly with children did not attend 16 hours of staff development activities annually.
1. The record for Staff #1 contained documentation of 10 of 16 required training hours of staff development activities.
2. The record for Staff #2 contained documentation of 14 of 16 required training hours of staff development activities.
3. The record for Staff #3 contained documentation of 2 of 16 required training hours of staff development activities.

Plan of Correction: I will make sure trainings are completed every year.

Standard #: 22VAC40-185-540-C
Description: Based on observation, the center's first aid kit did not include the minimum requirements.
The first aid kit was missing a second triangle bandage.

Plan of Correction: I will buy a triangle bandage.

Standard #: 22VAC40-185-550-M
Description: Based on record review, 1 of 3 injury reports did not contain all required information.
An injury report dated 4/3/2019 for Child #1 did not include the time parents were notified of the injury.

Plan of Correction: I will make sure there is always a time listed on the injury reports.
