A complaint regarding supervision, documentation, self-reporting and behavior guidance was received on June 4, 2019. The complaint was initiated on the same day. The investigation concluded on August 9, 2019. During the investigation staff and a child's record were reviewed. Additionally, staff were interviewed. The information gathered during the investigation partially supports the allegation. Therefore, the complaint is valid. Violations regarding center policy and behavior guidance were identified during the investigation and appear on the violation notice.
