Comments:
An unannounced monitoring inspection was completed on October 10, 2019 from approximately 9:30 a.m. to 12:00 p.m. Sixty-five children were in care and twelve staff and one volunteer were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include outside play, snack time, circle time, center time, structured learning time, and transitioning to different activities. Interviews and interactions were conducted and medications were reviewed. Six children?s records and five staff records were reviewed. The director was available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records and interview, the licensee failed to ensure that staff obtain and submit results of a follow-up tuberculosis screening a least every two years for one of five staff.
Evidence: 1. Staff #5's record, hired on September 5, 2017, contained tuberculosis screenings/tests dated June 19, 2017 and August 14, 2019, and therefore not within the required two year time frame. 2. Administrator #1 acknowledged the follow-up screening was not completed with the required time frame.

Plan of Correction: The director will let staff know that although they are not on premises during the summer months, if a tuberculosis screening/test is due it will have to be obtained within the required time frame.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records and interview, the licensee failed to maintain all the elements of staff?s records for one of five staff.
Evidence: 1. A review of Staff #2's resume, hired on September 3, 2019, as a program leader, indicated she holds an eligible college degree for the job position. However, there was documentation in the record of such education. 2. Administrator #1 acknowledged documentation of Staff #2's education was not obtained as required.

Plan of Correction: The director requested that Staff #2 submit her college transcripts on site.

Standard #: 22VAC40-185-210-A
Description: Based on review of staff records and interview, the licensee failed to ensure that a staff possessed all the required qualifications for a program leader for one of five staff.
Evidence: 1. Staff #4's record, hired on September 5, 2017 as an assistant and then promoted to program leader on September 9, 2019, lacked documentation of the required twenty-four hours of training six months prior to beginning work as a program leader. The record reflected sixteen hours of training before being promoted. 2. Administrator #1 acknowledged that Staff #4 did not complete the required amount of training.

Plan of Correction: Staff #4 is already scheduled to obtain training and therefore director will apply this towards the required qualifications.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of staff records and interview, the licensee failed to request within 30 days of hire or follow up on within 45 days of making the request an out of state child abuse and neglect search for one of five staff.
Evidence: 1. A review of Staff #3's sworn disclosure, hired on September 10, 2018, indicated Staff #3 has resided in another state in the past five years. However, the record did not contain an out of state child abuse and neglect search request for that state. 2. Administrator #1 acknowledged this search request had not been requested as required.

Plan of Correction: The director will submit documentation to licensing that the search has been requested within 10 business days of this notification.
