Comments:
An unannounced complaint inspection was conducted in response to a complaint received in the Licensing Office. Interviews were conducted in on 8/1/2019 and 8/15/2019 .Interviewed staff relating to the allegation of staff to child ratio . The preponderance of the evidence gathered during the investigation supports the allegation, so the complaint is determined to be "valid" .

Evidence:
1. Observed child's # 5 sleeping on a cot that was frayed at both ends of the cot. The child was laying on the floor. The frayed ends could not hold the child up in a manner that prevented him from laying of the floor.

Description: Based on observation, record review and interview, the center failed to ensure that was a staff member in each grouping of children who met the qualifications of a program leader regularly present.

Evidence:
1 Staff # 3 who was observed as the infant room teacher with a group of seven infants and toddlers on the afternoon of the inspection. The staff person did not have the required education and or training for program leader. The record of staff #3 did not have a documented job position. The director/ owner and staff #3 verified that staff #3 is the program leader who is regularly present with children.

Description: Based on observation, the center failed to ensure that all required staff to children ratio was met for one staff to eight two year olds.

Evidence:
1. There were a total of 19 children down stairs with two staff. One- 22 month old, four- two year old children, eight- preschool children and six-school age children present. Upon arrival staff were working on getting the children to rest for nap time. There were seven children awake. The children were jumping, rolling around the floor, yelling and playing. Approximately five minutes after the inspectors arrived a third staff person arrived.
