Comments:
An unannounced monitoring inspection was conducted from 8:45 am - 3:15 pm with the center director and staff. There were 56 children, ranging from five months to five years of age, supervised by eight staff. Children were observed participating in structured center play, completing an arts and craft activity, playing on the playground, eating lunch, and napping. Lunch served today was tomato rice soup with beef and vegetables, apple sauce and milk. Six children's records and five staff records were reviewed. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of six children's records, the center failed to ensure that each child has a physical examination before the child's first attendance or within one month after attendance. Evidence: the record for child #5 (date of first attendance: 8/19/19) did not have documentation of a physical.

Plan of Correction: Will ask the parents again to provide documentation of a physical.

Standard #: 22VAC40-185-60-A
Description: Based on a review of six children's records, it was determined that not all records contained all required information. Evidence: Child #3 and child #5's records' did not have the addresses for two emergency contacts. Child #2's record did not have the home address for the primary contact.

Plan of Correction: Will obtain the missing information.

Standard #: 22VAC40-185-70-A
Description: Based on a review of five staff records, it was determined that not all staff records had all required information. Evidence: the records for staff #1 did not contain written information to demonstrate that the individual possesses the education, orientation training, staff development, certification, and experience required by the job position. The record for staff #2 did not have documentation of orientation training.

Plan of Correction: Will obtain the missing information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, and staff interviews, it was determined that not all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. In both the pre-k and school age classrooms, one of the two toilets are clogged. Staff stated this issue has been ongoing, and they have been fixed several times in the past month. 2. In the pre-k classroom, one of the cots is caved in, causing it to touch the floor when a resting child is on it. 3. In the pre-k room, a couch has severe tears causing it to be absorbent. 4. On the playground, the surrounding fence has sharp links at the top, and disrepair in several places creating space for a child to slip through at the bottom. 5. On the playground, there is a timber tie piece of wood with a stake approximately 1 inch in length protruding out. 6. On the playground there is a bolt missing from the motorcycle bouncy equipment. 7. On the playground equipment, there is extensive rust on the steps.

Plan of Correction: Will work with maintenance to fix all of these issues.

Standard #: 22VAC40-185-280-B
Description: Based on observation, not all hazardous substances were kept in a locked place. Evidence: 1. In the toddler classroom, there was a bottle of sanitizing solution sitting out on top of the diaper changing table. 2. In the school age classroom, there were 5 spray bottles of sanitizing solution sitting in an unlocked cabinet.

Plan of Correction: The cabinet in the school age classroom was locked during the inspection.
