Comments:
The Licensing Inspector conducted an investigation in response to a self reported incident regarding a child found unsupervised in the hallway on September 20, 2019. Interviews were conducted with staff, and staff and child records reviewed. The information gathered during this investigation supports violations being cited that were discussed with the center director. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records did not include all required information.
Evidence:
1. Staff #1's file was missing verification of age requirement, job title, and date of employment. Staff #5 estimated Staff #1's date of hire (DOH) to be 6/04/2019.
2. Staff #2's (DOH 4/26/2016) file was missing documentation that two or more references were checked before employment.

Plan of Correction: Documentation titled "Change of Position" added to file."

Verification of age requirement filed with HR.

References completed.

Standard #: 22VAC40-185-240-A
Description: Based on record review and interview, all staff did not receive training by the end of their first day of assuming job responsibilities.
Evidence:
1. Staff #1 (date of hire (DOH) approximately June 4, 2019) and Staff #2 (DOH 4/26/2016) did not have documentation in their files that an orientation training had been completed including the staff members job responsibilities, the center's policies and procedures, playground safety procedures, recognizing child abuse and neglect and confidential treatment of personal information.
2. Staff #5 was unable to demonstrate that Staff #1 and 2 had completed orientation training.

Plan of Correction: Orientation training documentation added to file.

Standard #: 22VAC40-185-240-C
Description: Based on record review, staff working directly with children did not all complete 16 hours of training.
Evidence:
1. Staff #2's record did not have documentation of any training completed since hired 4/26/2016.

Plan of Correction: Effective immediately, the camp director and/or designee will submit new hire and training documentation yearly.

Standard #: 22VAC40-185-340-A
Description: Based on multiple staff interviews and documentation review, the center failed to ensure the care and protection of all children in care.
Evidence:

1. The center's incident report for Child #1 documents that on September 20, 2019 at approximately 4:00pm, Child #1 (4 year old) was left behind in the classroom during the class's transition to the playground. Child #1 was found alone and unsupervised in the hallway by Staff #3. Staff #3 walked the child outside to the playground to join the child's class.

2. Staff #1 and Staff #2 stated that during the transition from the classroom to the playground, Staff #1 and Staff #2 took 10 of the 12 children to the playground. Both staff thought that two children (Child 1 and Child 2) were with Staff #4. Staff #4 stated that during the transition to the playground, she stayed behind to help Child #2 with their jacket; that Child #1 was not with her.

3. Staff #1, Staff #2 and Staff #4 stated they did not communicate with each other about Staff 1 and 2 taking 10 children and Staff #4 taking 2 children.

4. Staff #2 estimated that the child was brought outside to join the class approximately 10-15 minutes after the class arrived on the playground.

5. Staff #1 and Staff #2 confirmed being unaware that Child #1 had been left behind and unsupervised until Staff #3 brought the child to the playground.

Plan of Correction: Each teacher will give verbal confirmation to the other(s) as to the number of student(s) they each have in their care.

Teachers will verbally confirm with each other that hot spots have been cleared and checked.

Standard #: 22VAC40-185-340-F
Description: Based on multiple staff interviews and incident report review, children under 10 years of age were not always within actual sight and sound supervision of staff.
Evidence:
1. The center's incident report for Child #1 documents that on September 20, 2019 at approximately 4:00pm, Child #1 (4 year old) was left behind in the classroom during the class's transition to the playground. Child #1 was found alone and unsupervised in the hallway by Staff #3. Staff #3 walked the child outside to the playground to join the child's class. Child #1 was not within actual sight and sound supervision for an undetermined length of time..

2. Staff #1 stated that during the transition from the classroom to the playground, Staff #1 and Staff #2 lined up 10 of the 12 children and took them to the playground. She stated "I thought Staff #4 stayed back with the other two children." Staff #1 stated they (Staff 1, 2 and 4) did not communicate with each other about Staff 1 and 2 taking 10 children and Staff #4 taking 2 children.

3. Staff #2 stated that Staff #1 and Staff #2 left the classroom with 10 children to go to the playground and thought during the transition that Staff #4 had the other 2 children (Child #1 and Child #2) from the class. Staff #2 confirmed being unaware that Child #1 had been left unsupervised. Staff #2 stated that Child #1 was brought to the playground to join the class by Staff #3 approximately 10-15 minutes after the class arrived on the playground.

4. Staff #4 stated that during the transition from the classroom to the playground, she stayed back in the classroom with Child #2 to help the child with their jacket. Staff #4 stated there was no communication with the other two teachers that she would keep Child #1 and Child #2 with her. Staff #4 stated she then went to the Galley to retrieve a bleach bottle, stopped by the office and then headed to the playground with Child #2. Staff #4 stated as she was coming out to the playground with Child #2, Child #1 was brought to the playground to join the class by Staff #3. Staff #4 was unaware that child #1 had been left alone. Staff #4 stated she thought the child was with the rest of the class. She estimated the child was alone approximately 1 or 2 minutes before Staff #3 found the child.

5. Staff #3 stated she saw Child #1 running in the hallway. Child#1 told Staff #3 his class had left him and that they were outside. Staff #3 stated she then walked the child out to the playground to join the class.

Plan of Correction: Teachers will do a name to face recognition before, during and after the transition is completed.

CDC conducted a staff training on transitions which included updated procedures and role playing.

Administrators will perform informal observations of transitions between activities and give feedback as necessary.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of four staff records and interview, the center did not obtain a central registry report within 30 days of employment for a staff member.
Evidence:
1. Staff #5 estimated that Staff #1's date of hire to be June 6, 2019.
2. Staff #1 did not have a central registry report on file.

Plan of Correction: Documentation titled "Change of Position" added to file.
