An unannounced focused inspection was completed on October 15, 2019 from approximately 10:00 a.m. to 11:45 a.m. to review compliance of specific standards related to the issuance of a provisional license. Twenty-three children were in care and eight staff were on premises during the inspection. Staff and children were observed engaged in activities to include activities for infants, free play time, structured learning time and lunch time. Interviews and interactions were conducted and four children?s records and three staff records were reviewed. The owner/director was present at the exit interview at which time inspection findings were reviewed. See the violation notice on the Department?s public web site for violations of the Standards.

Standard #: 22VAC40-185-270-A Description: Based on observation of classrooms, the licensee failed to ensure that areas and equipment of the center are maintained in a clean, safe and operable condition.
Evidence: 1. A plastic blue basket was observed in a classroom with several cracks, creating jagged, sharp edges. An unmounted television was observed in a classroom placed on top of a plastic shelf that shook to the touch and therefore creating an unsafe falling hazard. An area on a lower wall in a classroom was observed with an area of peeling paint. All of these hazards are accessible to children in care. 2. Administrator #1 acknowledged these hazards.Plan of Correction: The owner/director corrected the broken basket on site by moving it to a place inaccessible to children. The owner/director contacted a painter who will repair the spot on the wall by 10/21/19. The owner/director will have someone mount the television by 10/29/19.
