Comments:
An unannounced monitoring inspection was conducted today. There were 109 children in care, with a total of 19 staff members assigned to their direct supervision. Children's records, new staff records, classroom programming, the physical plant, children's medications, staff training, background checks, and emergency preparedness items were inspected today.

The children were observed playing group games in the multi-purpose room, eating lunch, enjoying circle time discussions, and listening to stories.

Violations:
Standard #: 22VAC40-185-150-A
Description: Based on a review of children's health records, it was determined that one record does not include a fully completed current Virginia Department of Health form to record the child's immunizations.
Evidence:
The immunization record for child A does not include the physician's signature to indicate that the child is adequately immunized for his/her age.

Plan of Correction: The parent will be asked to supply a shot record with the required statement signed.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that the children's records do not contain all required information.
Evidence:
All children's records reviewed do not include each parent's work phone number.

Plan of Correction: The form will be updated and the parents will be asked for all required information.

Standard #: 22VAC40-185-290-3
Description: Based on observations, it was determined that electrical outlets did not have protective covers as required.
Evidence:
In the multi-purpose room, there were 3 electrical outlets that were not covered. The Inspector observed two groups of preschool children using this classroom during the inspection.

Standard #: 22VAC40-185-560-M
Description: Based on observations, it was determined that staff did not sit with children during lunch time.
Evidence:
In Room 5, the Inspector observed the children while they were eating lunch. All children were seated at the tables, but the two staff in the classroom were observed standing in the classroom.

Plan of Correction: All staff will be reminded to sit with the children during lunch time.
