Comments:
A Subsidy Health and Safety inspection was conducted on October 8, 2019 to review supplemental health and safety requirements. This on-site visit was conducted from 11:40 a.m. through 1:40 p.m. There were sixty-four children in the care of twelve staff. No medication is being administered at this time. Seven children and four staff records were reviewed and updated. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Areas observed were: educational center activities, lunch, outside play, hand washing/diapering procedures and nap. Interactions between the children and staff were observed as appropriate and the children seemed comfortable with their daily routine and schedule.

Description: Based on documentation, the center failed to report a serious injury to the department within two business days.

Evidence: 1. An injury report was received by the department on August 8, 2109. 2. One child was injured, July 9, 2019, on the outside playground. 3. The center called the child?s guardian and the child was taken to the doctor. 4. The child's received a deep cut on his lip. 5. The center self-reported the incident on August 8, 2019.

Plan of Correction: There was a miscommunication in the reporting of the injury. In the future, all serious injuries will be reported to within two business days.
