Discussed with site supervisor bathroom procedures and supervision.

Comments:
An unannounced monitoring inspection was conducted today from 2:50pm -5:15pm. There were 27 children directly supervised by 4 staff. The physical plant, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Staff files were reviewed on an earlier date through the County of Loudoun, Department of Parks, Recreation, and Community Services main office. Children were observed participating in group play, doing crafts, coloring, playing with building blocks, working on their homework, and playing outside. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Areas of non-compliance are identified on the Violation Notice

Description: Based on observation, cleaning supplies were not kept in a locked place.

Evidence:
In the storage room, two spray bottles of disinfectant were observed on top of a closet, unlocked and accessible to children.

Description: Based on observation and interview, it was determined that a substitute container of a cleaning solution was not labeled to indicate its contents.

Description: Based on observation, the program did not maintain a complete record of injuries.

Evidence:
Five injury/incident reports were reviewed during the inspection. Three reports were missing future actions to prevent recurrence of the event. Four reports were missing the time of notification. Two reports were missing documentation of how the parent was notified. Two reports were missing the date of notification. 1 report was missing the time of notification.
