Comments:
A Subsidy Health and Safety inspection was conducted on 10/03/2019 to review supplemental health and safety requirements. There were 57 children and 5 staff members present. The physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications, and the Emergency Preparedness Plan were reviewed. 5 children's records and Staff certification for CPR/First Aid, Mat training, and emergency information were also reviewed. Ratios were in compliance and required documents were posted. The children were observed participating in the following: handwashing, eating snack, completing homework, playing a variety of board games, and playing outside. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-530-4
Description: Based on record review and staff interview, the center did not ensure there was documentation that all staff members completed certification in first aid.
Evidence:
1. Documentation of competed first aid certification for Staff #1 was not available for review.
2. Staff #1 stated first aid certification was completed but they did not have documentation of completion on site at the time of inspection.

Plan of Correction: I will make a binder for substitute staff to keep all required information.

Standard #: 22VAC40-665-530-5
Description: Based on record review and staff interview, the center did not ensure there was documentation that training required in 22VAC-665-580 has been completed.
Evidence:
1. Documentation of completed Virginia Preservice Training for Child Care Staff for Staff #1 was not available for review.
2. Staff stated the training was completed but they did not have documentation of completion on site at the time of inspection.

Plan of Correction: I will make a binder for substitute staff to keep all required information.
