Comments:
An unannounced monitoring inspection was conducted on October 16, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at 11:00 am and concluded at 1:45 pm. Upon the inspectors arrival children in the preschool classroom were observed playing in centers that consisted of play-dough, blocks, house keeping and computer. The two year old class sponge painted and later played with instruments. The pre-K class worked on a halloween project making mummies with paper cut outs and tape. Toddlers worked with picture flash cards before preparing for lunch. Children's hands were washed and tables cleaned. Lunch time was observed in the infant room. Staff held infants with bottles. Staff interactions with the children were positive. The center has appropriate supplies and materials. All areas were inspected with the exception of the playground due to inclemant weather. The center's first aid kit and non-medical supplies were inspected. The inspector interacted with staff and children in care.

Five staff records and 10 children's records were reviewed. Medication and authorizations were reviewed. The center has staff with current CPR, first aid,daily health observation training and MAT.
The most recent evacuation drill was documented on 10/08/2019. The two required shelter-in-place drills were completed on 04/05/2019 and 10/04/2019.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of ten children's records, the center did not obtain all required information for one child's record.

Evidence:

The record of child #8, did not contain the address and phone number of a second person to be contacted in an emergency if a parent cannot be reached.

Plan of Correction: Corrected while the inspector was on site. The director contacted the parent and obtained the missing information.

Standard #: 22VAC40-185-510-N
Description: Base on review of medication and authorizations, the center did not ensure when an authorization for medication expired, the parent was notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

Evidence:

1. The medication authorization for child #11 expired on May 10, 2019. The medication was on site exceeding the 14 days allowed to return or dispose of the medication.
2. The medication authorization for child #12 expired on January 15, 2019. The medication was on site exceeding the 14 days allowed to return or dispose of the medication.
3. The medication authorization for child #13 expired on September 20, 2019. The medication was on site exceeding the 14 days allowed to return or dispose of the medication.

Plan of Correction: Per the director: I will return all the medications tonight to the parents.

Standard #: 22VAC40-185-540-E
Description: Based on observation, the center did not maintain one working, battery-operated radio in each building used by children.

Evidence:

The center did not have working batteries required to power the center's battery operated radio.

Plan of Correction: Per the director: I will purchase new batteries for the radio.
