Comments:
The licensing inspector conducted an unannounced monitoring inspection on 10/15/2019 from 10:02am to 1:05pm. The children were observed learning through play in centers, playing on the playground, brushing their teeth, eating lunch, and napping. A formal observation was completed with classroom #2 and early head start classrooms. Lunch consisted of chicken fried steak, mashed potatoes and gravy, baked apples, roll and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the center's mini bus.

Five children's records were reviewed. Staff records are kept off site and will be reviewed on a later date.

The center's first aid kit and emergency supplies were inspected.

Today, the following child to staff ratios were observed:
Rm. #1 (Threes and Fours) - 12:2

Toddlers - 8:3
Infants - 3:3


The violations from the previous inspections were checked for corrections. There were three repeat violations found during today?s inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure that areas and equipment of the center were maintained in a clean, safe and operable condition.
Evidence:
There were several areas throughout the center with chipped and peeling paint within the reach of children in care.

Plan of Correction: I will put in a maintenance request today and it will be completed by the 25th.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. The cafeteria had cleaners located in an unlocked cabinet out of the reach of children in care.
2. The center's bus had a cleaner located on the floor behind the driver's seat within the reach of children.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center did not ensure that electrical outlets had protective covers that are of a size that cannot be swallowed by children.
Evidence:
Classroom #1 had two uncovered outlets located on an extension cord on top of the hand washing sink within the reach of children in care.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not ensure that disposable diapers were disposed in a leak proof or plastic lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence:
The inspector observed soiled diapers in a trash can located in the early head start restroom. The trash can was not leak proof or foot operated.
