Technical Assistance:
Technical assistance was provided regarding reference checks for employees.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am-12:00pm. There were 59 children (infants-5yrs.) supervised by 13direct care staff. The physical plant, 7 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, circle time and coloring. Diapering and hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-185-60-A
Description: Based on review of 5 children's records, 1 was incomplete.
Child #1, start date 3/18/19, does not have documentation of emergency contacts or insurance information.

Plan of Correction: I will contact the parent today, going forward we will review all records prior to child beginning care.

Standard #: 22VAC40-185-70-A
Description: Based on review of 7 staff records, 1 is does not have documentation of 2 reference checks.
Evidence:
Staff #2 does not have documentation of 2 references.

Plan of Correction: Corrected during inspection, in the future we will contact references and document information.

Standard #: 22VAC40-185-350-A
Description: Based on observation, staff to children ratio was not maintained in the C classroom.
Evidence:
A staff member took 1 child to change a diaper and left another staff member alone with 9 children.

Plan of Correction: Staff will use the walkie talkie to contact another staff member to cover when they need to leave the room.

Standard #: 22VAC40-185-500-A
Description: Based on observation, children's hands were not washed after toileting.
Evidence:
A child's hands were not washed after a diaper change.

Plan of Correction: We will make sure all children's hands are washed after toileting.

Standard #: 22VAC40-185-500-B
Description: Diapering surface was not cleaned and sanitized according to licensing standards.
Evidence:
The diapering surface was only sprayed with a solution of bleach and water after a diaper change. The correct procedure is to spray the surface with a solution of soap and water, wipe dry, then spray with a solution of bleach and water and allow to air dry after a diaper change.

Plan of Correction: That is the procedure we follow, we will retrain all staff at our next meeting on Friday.

Description: Based on review of 6 staff records, 1 does not have a current Central Registry search.
Evidence:
Staff #1 has documentation of an expired Central Registry search, 3/10/19.

Plan of Correction: We have send in a new Central Registry search form and send the inspector documentation that it was completed.
