An unannounced monitoring inspection was conducted today from 12:00am-2:30pm. There were 11 children (3-4yrs.) supervised by 3 direct care staff. The physical plant, 2 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing in home living, having story time, and engaging freely. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Evidence: Staff A and Staff B's most recent documented TB screenings were dated 8/18/2017 (expired: 8/18/2019).Plan of Correction: We will ask staff to obtain new TB tests.

Evidence: Staff A's (date of hire: 8/11/2017) record did not contain documentation of completed reference checks.Plan of Correction: We will look for the completed reference checks in staff files.

Standard #: 22VAC40-185-560-G Description: Based on observation, 2 of 11 lunch bags brought from home were not clearly dated and labeled in a way that identifies the owner.
Evidence: In the Extended Day classroom (children ages 3-4 years), 2 children's lunch bags did not contain documentation of the date, and 1 child's lunch bag did not contain documentation of the child's name.Plan of Correction: We will remind parents to label food from home with name and date.
